Document delivery rules for payment of not received dividend from the year 2008

1. Contact the Central Depository and Clearing Company Inc. (CDCC) to determine if you really are entitled to dividend payment from the year 2008 (tel. SKDD: + 385 1 4607 300; skdd@skdd.hr).

2. If you are entitled to dividend from the above mentioned period, such payment will be made by Hrvatski Telekom d.d., and you need to submit the following documents:

  1. Copy of Identity Card (both sides) or Passport (picture page and signature page)
  2. OIB number
  3. Investor designation number („oznaka investitora“) from the CDCC (1) ,
  4. Copy of your personal current account, savings account or giro account card (both sides) or a copy of the contract on opening of one of the a.m. accounts
  5. Contact telephone number
    Please note that all data and documents shall be submitted only for the person stated as shareholder.
  6. In case of share/ dividend right inheritance, please also submit a copy of the final Resolution on inheritance and all above mentioned items stated under a, b, d, e as well as the late person’s OIB number and Investor designation number („oznaka investitora“) from the CDCC (1) .

3. If the shareholder is a minor, then you need to submit the following documents:

  1. Copy of the Birth Certificate together with a copy of the Identity Card (both sides) or Passport (picture page and signature page) of the parent or guardian
    or
    Copy of Identity Card (both sides) or Passport (picture page and signature page) of a minor
  2. Copy of the current account, savings account or giro account card (both sides) of the parent or guardian or a copy of the contract on opening of one of the a.m. accounts

  3. OIB number of a minor
  4. Investor designation number („oznaka investitora“) from the CDCC (1)
  5. Contact telephone number of the parent of guardian

(1) To be found on the note you received from the CDCC when your proprietary account was opened, or on the CDCC note on your proprietary account balance. If you are not in possession of such notes, please get in touch with the CDCC to receive the investor designation number.
